ISP Course Forms

ISP Prep Class Form

Each International Study Program (ISP) is required to provide a pre-departure preparation course for participating students. These courses are typically offered as a 1.0-credit course during the second block of Winter Semester.

Once Ecclesiastical Clearance Office (ECO) approval has been obtained, please complete this form to initiate the creation of your ISP preparation course.

Course Creation Form

The Course Approval Form must be completed to initiate the creation of courses associated with a study abroad program. Please follow the instructions below when completing the form:

  • List all courses that may be offered as part of the program. Include courses that are still under consideration, as this will help accommodate potential curriculum adjustments or last-minute changes.
  • If the program includes courses from multiple academic departments, a separate Course Approval Form must be completed and submitted for each department to obtain the required departmental approvals and signatures.
  • Once all required participants have completed their review and signed the form, it will automatically be uploaded into the system for processing and recordkeeping.

Participant Information Instructions: As the individual completing the form, you will be identified as Participant 1. You will be prompted to provide the required information for Participant 2 and Participant 3 as part of the approval workflow.

  • Participant 2: Department Chair responsible for reviewing and approving the course request.
  • Participant 3: Associate Dean responsible for the final review and approval.

Please ensure that the contact information entered for each participant is accurate to avoid delays in the approval process.
